The first thing you’ll be asked is whether you want to report with your contact details or submit the form anonymously. Information on the difference between the two is here.

Describe what has happened. There a list of behaviours to choose from. You can select multiple answers, and you can also choose ‘Other’. There is then an optional free text box so that you can provide more details if you wish, and you can also upload images such as screenshots. If you don't feel comfortable providing this, that's fine.

Select whether the incident happened to you or someone else. You can also choose 'prefer not to say'.

Select your relationship with the person you are reporting, where the incident(s) took place, and whether you are a student or staff member. For all of these, you can also choose 'prefer not to say'.

Optional: If you believe the incident was motivated by hostility or prejudice towards any protected characteristics, you can indicate these here.

If you are reporting with contact details, please provide these.

Optional: If you are reporting anonymously, please provide any reasons for this. There is a list for you to choose from, or you can choose 'Other' and enter free text.

If you are reporting anonymously, please choose whether you wish to opt in to anonymous messaging. More information about this is here.

Optional: Provide your demographic information. This helps us to understand who is reporting.

Optional: Let us know where you heard about Report and Support.

You will then have the opportunity to review and change any of your answers and submit your form.
